Complete Your Registration Forms

To save time during your appointment, you can download, print, and complete your registration forms at home. Bring the completed documents with you on the day of your visit.
If you prefer to fill out your forms in the office, please arrive at least 15 minutes early to allow enough time for completion.

What to Bring to Your Appointment

To make the most of your first visit and avoid any delays in your care, please bring the following with you:

Photo ID
A valid government-issued photo ID is required for all new patients.

Insurance Cards
Bring your dental and medical insurance cards so we can verify your benefits before your appointment begins.

Completed Registration Forms
If you downloaded and printed your forms in advance, bring the completed packet with you. If you haven't had a chance to fill them out, plan to arrive at least 15 minutes early to complete them in the office.

Current Medications List
A written list of all medications you currently take, including dosages, helps our team provide safe and informed care.

Referral Slip
If your dentist or physician provided a referral, please bring that documentation with you.

Recent X-Rays or Imaging
If you have recent dental or medical imaging, bring copies or ask your provider to send them to our office ahead of your visit.

New patients often have questions about forms, insurance, and what to expect. These FAQs provide quick, helpful answers before your first visit.

Completing your forms ahead of time saves time and helps us prepare for your visit, but you may also fill them out in the office. If choosing to complete your paperwork in the office, please arrive 15 minutes before your appointment time to avoid delaying your appointment.

We can still see you, but bringing your card ensures accurate benefit verification. Keep in mind, if we are unable to verify your insurance, you will be asked to pay the balance in full for all services rendered until you provide our office with the needed coverage information.

 

For security reasons, we recommend bringing printed copies unless our team has arranged an alternative method.

It is highly recommended that patients explore and discuss restorative options with their general dentist prior to seeking advice from an oral surgeon. Often a surgeon is working in conjunction with a patient’s general dentist to execute a specific comprehensive treatment plan for that patient and everyone needs to be on the same page.

If you have recent imaging, bring copies or have your dentist send them to our office. X-rays may be emailed to our office via the email address info@cherryorchardoralsurgery.com.
